Community Health Worker, Certificate of Achievement

Community Health Workers (CHWs) work with individuals and groups to promote health knowledge and self-sufficiency through outreach, community education, informal counseling, social support and advocacy. This Certificate of Achievement prepares students for an entry-level posisiton as Community Health Worker (level 1). Other position titles include: Promotore, Patient Navigator, Patient Advocate, Health Navigator, Health Ambassador, Lifestyle Coach, Community Advocate, and others. Note: Students who have completed MEDA 100, MEDA 101, MEDA 110, MEDA 112, MEDA 114, MEDA 120, and MEDA 121 only need HUMS 53 and MEDA 251 to complete the CHW Certificate.

Learning Outcomes

Upon successful completion of the program, students will be able to:

  • Identify basic health needs and assist in accessing resources for at-risk individuals and groups in the local community.
